HART (Homelessness, Addictions, Recovery & Treatment) Hubs transitioning from Consumption and Treatment Service (CTS) Sites

People experiencing homelessness, at risk housing situations, mental health challenges, or substance use often find the healthcare system difficult to navigate.

Crow Shield Lodge offers pathways support for self identified Indigenous and non Indigenous with a focus on Indigenous Traditional Land-based alternative healing.
The Hart Hub Waterloo Region was established to provide personalized guidance, resources, and support to address each individual’s needs. It also ensures people have the information they need to make informed choices about their health and housing. With this support, individuals can feel more connected, empowered, and confident in accessing the right care at a time that works for them.
